Six Steps to Get Started

with 

The L.E.A.P. Program

1.

All dogs attending the L.E.A.P. program must:

 

— Be neutered or spayed by seven months old.

— Meet Shoreline Pet Lodge’s vaccination requirements.

— Possess a copy of a current [Connecticut town] Dog License.

(Not required for dogs under six months old.)

2.

All dogs are required to be treated with a veterinarian-approved flea and tick prevention program.

It is also highly recommended that all dogs receive monthly heartworm prevention treatment.

3.

Please have your veterinarian send your pet’s spay/neuter, vaccination, and town dog license documents to Shoreline Pet Lodge via fax (315.4892) or email at info@shorelinepetlodge.com.

4.

Complete the Shoreline Pet Lodge Enrollment Form and send it to us via fax (315.4892) or email (info@shorelinepetlodge.com).

5.

Upon receiving a copy of your dog’s license, veterinary records, and completed enrollment form — please stay tuned!

6.

Shoreline Pet Lodge will call you to schedule an appointment to meet and evaluate your four-legged friend.
